Join MSA in Brooklyn for a visit to the Masjid Al Aman Inc. Masjid Al Aman is a place of worship, a landmark of communities in the surrounding neighborhoods of East NY and Ozone Park area. Open every day to Muslims for 5 times daily prayer, Jumma on Fridays, Muslim holiday (Eid) and community events.

Students will meet in Dogwood Parking lot at 7AM on Friday, September 13 to board the bus to Brooklyn. After visiting the Mosque, the bus will take the group to ZamZam Halal for lunch. Students should bring funds for lunch. Menu items range in price from $7.99 - $12.99 with a few select items priced higher.

ASSUMPTION OF RISK, LIABILITY RELEASE, INDEMNIFICATION AND WAIVER
FOR PARTICIPATION IN VOLUNTARY ACTIVITY AND TRAVEL

This is a legally binding Release and Waiver. I, ___________________________, desire to participate in the following Salisbury University (“University”) voluntary activity: ______________________________________ __MSA Trip to Masque___ on _____Sept. 13, 2024__________ (dates) (“Activity”). I understand and agree that I may be transported in a University vehicle or in a private vehicle operated by University staff or students. I fully understand and appreciate the dangers, hazards and risks inherent in the Activity and in the transportation to and from the Activity, which dangers include but are not limited to physical or mental injury or death. I understand that University personnel may accompany the Activity, but that they are not acting as a chaperone and I will be free to engage in the Activity without University supervision.  
1.	Assumption of Risk and Waiver of Liability: I, individually, and on behalf of my heirs, successors, assigns and personal representatives, acknowledge and fully understand that I will be engaging in activities that include the risk of injury or death resulting not only from my actions, inactions or negligence, but the actions, inactions or negligence of others. These risks include, but are not limited to, equipment used during the activity, conditions that exist on the premises, and other risks that may not be known to me or are not reasonably foreseeable. I understand and knowingly agree to assume the risk for participation in such activities.  I hereby release and forever discharge the University, the University System of Maryland (“USM”), the State of Maryland and its employees, agents, officers, trustees and representatives (“Releasees”) from any and all liability whatsoever for any and all damages, losses or injuries (including death), I sustain to my person or property or both, including but not limited to any claims, demands, actions, causes of action, judgments, damages, expenses and costs, including attorneys’ fees, which arise out of, result from, occur during or are connected in any manner with my participation in the Activity and/or any travel incident thereto, except for such damages or injury as may be caused by the gross negligence or actual malice of Releasees. 

2.	Statement of Indemnification:  I understand that, although the University has made every reasonable effort to assure my safety while participating in the Activity, there are unavoidable risks and I hereby voluntarily agree on behalf of myself and my heirs, successors, assigns and personal representatives, to indemnify, defend and hold harmless the University, the USM, the State of Maryland and its employees, agents, officers, trustees and representatives (in their official and individual capacities) from any and all liability, loss, damage or expense, including attorneys’ fees, that they or any of them incur or sustain as a result of any claims, demands, actions, causes of action, damages, judgments, costs or expenses, including attorneys’ fees, which arise out of, occur during, or are in any way connected with my participation in the Activity or any travel incident thereto unless due to the gross negligence or actual malice of Releasees.

3.	Disciplinary Actions: Similarly, I understand that should my conduct violate any state or federal law, the Student Code of Conduct, University personnel regulations or any policy of the University or the USM, I may be required to leave the Activity at the sole discretion of the University’s representatives and agents. Such conduct may result in legal and/or student conduct/personnel action.

4.	I certify that I am fully capable of participating in the Activity without causing harm to myself or others. I acknowledge that I am solely responsible for my own safety and well-being, as well as any injuries, property damage or loss during the Activity including, but not limited to, travel to and from the Activity. I am hereby advised and understand that it is my sole responsibility to decline, decrease or cease participation in the Activity if I am unable to participate due to lack of training, illness, injury or other medical condition. 

5.	I agree to report to appropriate University officials any condition I have that may require medical attention or accommodation at least seven (7) days prior to departure. I understand that the University will not provide health or accident insurance and I am hereby advised that I may acquire my own insurance. I understand that it is solely my responsibility to seek and receive medical evaluation and treatment for any symptoms that may arise during the course of participation in the Activity and that the University does not have medical personnel available at the location of the Activity. I grant the University permission to authorize emergency medical treatment, if University personnel is present, and if necessary. I understand and agree that the University assumes no responsibility for any injury or damage that might arise out of or in connection with such authorized emergency medical treatment.  

6.	The right is reserved by the University, in its sole discretion, to cancel the Activity (if within the University’s control) or any aspect thereof if the University determines or believes that any person is or will be in danger if the Activity or any aspect thereof is continued.

7.	I agree that this Assumption of Risk, Liability Waiver, Release and Indemnification Agreement is to be construed, interpreted and enforced under the laws of the State of Maryland and any dispute shall be adjudicated in a court of competent jurisdiction in Wicomico County, MD; and that if any portion hereof is held invalid, the balance hereof shall, notwithstanding, continue in full legal force and effect.

8.	Waiver of Legal Rights:  I have signed this Agreement in full recognition and appreciation of the dangers, hazards and risks of such activities, and I represent that my participation in the Activity is wholly voluntary. By signing this document, I hereby acknowledge that I have read this entire document, that I understand its terms, that by signing it I am giving up substantial legal rights I might otherwise have, and that I have signed it knowingly and voluntarily. I certify that I am at least 18 years of age and that this form is a condition of participation in the Activity.
